A current physical must be on file with Lompoc High School in order for any student athlete to participate in any PRACTICE of any sport.

LHS sports physical paperwork can be obtained at LVMS, VMS, La Purisima Catholic school and in the Lompoc High School athletic department prior to the physicals given on May 21, 2016.  All paperwork must be filled out, signed and presented at the time of entrance to the physicals - parental signatures are required.

Physicals will be $5.00 payable on May 21, 2016 - the physicals will take place at the office of Dr. Michael Gill located at 1025 East Ocean Avenue, on the corner of Ocean and Third streets.

Physicals obtained at your personal physician are always accepted, please pick up paperwork for your medical physician prior to your visit.
